I'm not Rich (who indeed writes great stuff, thanks!), 
but I would start with seeing whether you could
get a public, static IPv4 address from your Internet Service 
Provider (this is what I do).

If you can't, but have spare rackable hardware I would
look into finding a suitable cheap colocation space to
host it (this I what I do).

If you can't, I'd look into renting physical hardware in
a suitable jurisdiction (this is what I used to do).

Next step would be a virtual server in a suitable
jurisdiction (e.g. we picked Iceland).

Further steps would depend on answers to above questions.
